Employer Groups: If you have a group with 2 or more employees that would be interested in this coverage, complete the following questionnaire and we will provide you with group rates. Rates will be lower if the employer contributes towards the premium.

Contributory- the employer can select how much of the premium to pay but must, at a minimum, contribute 50% of the Employee Only rate.
Voluntary - the employees pay 100% of all premiums.
